the ways CHOC Health Alliance does this is by telling you about your privacy rights
as a member of CalOptima. Please read the CalOptima "Notice of Privacy Practices"
below. It tells you how CalOptima uses and gives out your health information. It
also explains CalOptima's duties and your rights. Please call CalOptima's Customer
Service Department at 1-888-587-8088 with questions. If you have trouble hearing,
